The COVID-19 Test Center at Tolleson Veterans Park is closed. COVID-19 home test are available at the Tolleson Public Library, while supplies last.
Health Clinic at Tolleson Civic Center
The Tolleson Fire Department, and Maricopa County Public Health, will be hosting a vaccine clinics at the Tolleson Civic Center.
The events are held at the Tolleson Civic Center, 9055 W. Van Buren St. - See map
Schedule:
- Tuesday, November 14th, 1:00 p.m. - 5:30 p.m.
- Tuesday, December 12th, 1:00 p.m. - 5:30 p.m.
Vaccines we will be offering:
- COVID-19 (Moderna)
- Flu
- Hepatitis A & B
- HPV (11 to 45 yrs. old)
- Tdap (tetanus)
- MMR
- Varicella
- Shingles (50 +) - *Limited supply
- Pneumococcal (65 +) - *Limited supply
Walk-ins are welcomed but appointments are encouraged to help plan the amount of staff needed. To schedule an appointment please call 602-506-6767.

As an alternative, COVID-19 home tests are now available at the Tolleson Public Library, while supplies last:

The operating hours for COVID-19 home test distribution at the Tolleson Public Library:
- Monday - Wednesday:** 9:00 a.m. to 7:00 p.m.

- Saturday:** 9:00 a.m. to 1:00 p.m.
- Sunday:** Closed
City Hall is open.
